    Milligan Morgan Haemorrhoidectomy vs LigaSure Haemorrhoidectomy : Comparative Postoperative Outcomes

    Objective: To compare the traditional Milligan Morgan haemorrhoidectomy with haemorrhoidectomy using LigaSure in terms of postoperative complications, patient satisfaction and hospital stay. Methodology: This is a randomized controlled trial carried out at the Department of Surgery Liaquat university hospital Jamshoro from July 2017 to June 2019. A total of 88 patients were admitted with the diagnoses of 3rd and 4th degree haemorrhoid were included in the study. Patients were randomly divided into two groups by lottery method. Group A underwent Milligan Morgan Haemorrhoidectomy and group B underwent Haemorrhoidectomy by Ligasure after the informed consent. Outcomes of both procedures were also compared by complications, patient satisfaction and hospital stay. Results: Out of 88 patients 35 were male (39.78%) and 53 were female (60.22%). The most common group of age involved was between 35–55 years. Third degree Haemorrhoids were present in 40 (45.45%) of patients while the remaining 48 (54.55%) had fourth degree Haemorrhoids. Group A included 44(50%) cases while Group B included 44 (50%) cases. The mean operating time of Group A was 50.5 (minutes) with a standard deviation of 11.5 while it was 35.5 ± 9.4 in B group. The mean blood loss in group A was 65.30 ml with a standard deviation of 14.58 while it was 45.45 ml ± 20.49 in group B. Conclusion: The Haemorrhoidectomy done by Ligasure is comparatively better than the Milligan Morgan Heamorrhoidectomy, in terms of operative time, less bleeding, less pain, less hospital stays and early return to work

    Application of Lateral Wedge in Knee Osteoarthritis for Improving Pain and Quality of Life

    OBJECTIVE: To determine the effectiveness of lateral wedge with and without conventional physiotherapy in relieving knee pain and quality of life in patients of knee Osteoarthritis. METHODOLOGY: This Randomized controlled study was conducted in Madina Clinics, Faisalabad. By using simple random technique 40 patients in range 40-60 years of age with clinically and radiologically diagnosed cases of grade II and III were studied, from February to August 2016. Subjects were divided in two groups with 20 subjects in each group. KOOS knee survey was used for the assessment of Quality of life. RESULTS: Mean total pain score on baseline of experimental group was 79.95±2.5 while the score amongst the control group was 79.95±7.69, after application of lateral wedges along with other treatments, score after 2nd week was 84.65 ± 2.94 while in control group was 80.25±2.9.97. After 4th week the symptoms score in experimental group was 83.00 ± 0.00 while control group was 88.95 ± 2.96. After the 6th week score was 91.5±14.5 of experimental and 81.8±9.5 of control group.These results of insole Wedges is also correlated with a lateral shift in the location of the COP in stance phase. Increase in the knee joint varus moment with insole Wedges suggests that the indication and limitations of laterally wedged insoles in general should be analysed in detail. CONCLUSION: This study concluded that the lateral wedges were significant in reduction of pain and symptoms as well as improve the quality of life

    Effects of Swirl Bubble Injection on Mass Transfer and Hydrodynamics for Bubbly Flow Reactors: A Concept Paper

    Bubble flow reactors (BFR) are commonly used for various industrial processes in the field of oil and gas production, pharmaceutical industries, biochemical and environmental engineering etc. The operation and performance of these reactors rely heavily on a range of hydrodynamic parameters; prominent among them are geometric configurations including gas injection geometry, operating conditions, mass transfer etc. A huge body of literature is available to describe the optimum design and performance of bubbly flow reactors with conventional bubble injection. Attempts were made to modify gas injection for improved efficiency of BFR's. However, here instead of modifying the geometry of the gas injection, an attempt has been made to generate swirl bubbles for gaining larger mass transfer between gas and liquid. Here an exceptionally well thought strategies have been used in our numerical simulations towards the design of swirl injection mechanism, whose paramount aspect is to inhibit the rotary liquid motion but facilitates the swirl movement for bubbles in nearly stationary liquid. Our comprehension here is that the swirl motion can strongly affect the performance of bubbly reactor by identifying the changes in hydrodynamic parameters as compared to the conventional bubbly flows.     Reemergence of Epidemic Vibrio cholerae O139, Bangladesh

    During March and April 2002, a resurgence of Vibrio cholerae O139 occurred in Dhaka and adjoining areas of Bangladesh with an estimated 30,000 cases of cholera. Patients infected with O139 strains were much older than those infected with O1 strains (p<0.001). The reemerged O139 strains belong to a single ribotype corresponding to one of two ribotypes that caused the initial O139 outbreak in 1993. Unlike the strains of 1993, the recent strains are susceptible to trimethoprim, sulphamethoxazole, and streptomycin but resistant to nalidixic acid. The new O139 strains carry a copy of the Calcutta type CTXCalc prophage in addition to the CTXET prophage carried by the previous strains. Thus, the O139 strains continue to evolve, and the adult population continues to be more susceptible to O139 cholera, which suggests a lack of adequate immunity against this serogroup. These findings emphasize the need for continuous monitoring of the new epidemic strains

    © 2020 Elsevier Ltd Chromium (Cr) poses serious consequences on human and animal health due to its potential carcinogenicity. The present study aims at preparing a novel biochar derived from Chenopodium quinoa crop residues (QBC), its activation with magnetite nanoparticles (QBC/MNPs) and strong acid HNO3 (QBC/Acid) to evaluate their batch and column scale potential to remove Cr (VI) from polluted water. The QBC, QBC/MNPs and QBC/Acid were characterized with SEM, FTIR, EDX, XRD as well as point of zero charge (PZC) to get an insight into their adsorption mechanism. The impact of different process parameters including dose of the adsorbent (1–4 g/L), contact time (0–180 min), initial concentration of Cr (25–200 mg/L) as well as solution pH (2–8) was evaluated on the Cr (VI) removal from contaminated water. The results revealed that QBC/MNPs proved more effective (73.35–93.62-%) for the Cr (VI) removal with 77.35 mg/g adsorption capacity as compared with QBC/Acid (55.85–79.8%) and QBC (48.85–75.28-%) when Cr concentration was changed from 200 to 25 mg/L. The isothermal experimental results follow the Freundlich adsorption model rather than Langmuir, Temkin and Dubinin-Radushkevich adsorption isotherm models. While kinetic adsorption results were well demonstrated by pseudo second order kinetic model. Column scale experiments conducted at steady state exhibited excellent retention of Cr (VI) by QBC, QBC/MNPs and QBC/Acid at 50 and 100 mg Cr/L. The results showed that this novel biochar (QBC) and its modified forms (QBC/Acid and QBC/MNPs) are applicable with excellent reusability and stability under acidic conditions for the practical treatment of Cr (VI) contaminated water

    Search for the standard model Higgs boson decaying into two photons in pp collisions at sqrt(s)=7 TeV

    A search for a Higgs boson decaying into two photons is described. The analysis is performed using a dataset recorded by the CMS experiment at the LHC from pp collisions at a centre-of-mass energy of 7 TeV, which corresponds to an integrated luminosity of 4.8 inverse femtobarns. Limits are set on the cross section of the standard model Higgs boson decaying to two photons. The expected exclusion limit at 95% confidence level is between 1.4 and 2.4 times the standard model cross section in the mass range between 110 and 150 GeV. The analysis of the data excludes, at 95% confidence level, the standard model Higgs boson decaying into two photons in the mass range 128 to 132 GeV. The largest excess of events above the expected standard model background is observed for a Higgs boson mass hypothesis of 124 GeV with a local significance of 3.1 sigma. The global significance of observing an excess with a local significance greater than 3.1 sigma anywhere in the search range 110-150 GeV is estimated to be 1.8 sigma. More data are required to ascertain the origin of this excess.Comment: Submitted to Physics Letters
